Maxine Lee Coultas, 69, of Niles, passed away on Thursday, Oct. 29, 2015, at her home.

Maxine was born on June 2, 1946 to the late Richard and Mary Lewis in Chicago, Illinois. In 1970, she married Alan Coultas in Roseland, Indiana.
She is survived by her husband of 45 years, Alan; sons, Christopher and Matthew Coultas, both of Niles; daughter, Amanda Van De Putte of Niles, and six grandchildren.
